Neurologic Complications in Renal Transplant Recipients




Abstract
As the renal transplantation is rapidly becoming a standard treatment of ESRD, a varieties of neurologic complications in renal allograft recipients are being encountered in Korea. How-ever, the incidence and clinical features has not been well addressed yet. Therefore, the authors attempted to assess the neurologic problems in 742 renal allograft recipients, who had been managed at Yonsei University Medical Center from 1979 to 1991. The following results were obtained:
1) Overall incidence of neurologic complications was 7.4%. The categories of theses were uremic and non-uremic metabolic derangements, strokes, CNS infection, and Cyclosporine toxicity in decremental order.
2) In terms of the temporal profiles of the neurologic complications, Cyclosporine toxicity mostly occurred within 10 postoperative days. Strokes were most frequently encountered with-in 12 months after the operation. Metabolic derangements were rather scatterd throughout acute and chronic stages, regardless uremic and non-uremic etiologies.
3) Twenty three out of 51 patients eventually died of the neruologic complications, thus implicating a grave prognostic factors. Particularly, fungal CNS infection and combined metabolic encephalopathy were thought to be the poor prognostic entities.
